Clinical Case Challenge XLUNG Patient with Amyotrophic Lateral Sclerosis (ALS) and Problems with Non-Invasive Ventilation (NIV)

By: Marcelo Alcantara, Médico - 04/03/2025 15:06

👉 A 49-year-old patient with Amyotrophic Lateral Sclerosis (ALS) without bulbar involvement presented with pneumonia and hypercapnic respiratory failure.

👉 Non-invasive ventilation (NIV) was initiated using a portable ventilator and an oronasal mask. The following mechanical ventilation (MV) curves are presented below, along with a corresponding video.

👉 The ventilator is set to BiPAP S/T mode, with a trigger sensitivity of 3 L/min and cycling set at 33% of peak flow.

🤔 What asynchronies and mechanisms are involved?

🤔 What course of action do you recommend?
