Proper adjustment of tidal volume (VT) alarms is essential to ensure safety and effectiveness during mechanical ventilation, especially when managing patients under a lung-protective strategy in pressure-controlled ventilation (PCV). This exercise invites you to analyze a realistic scenario and apply clinical reasoning to set appropriate VT alarm limits.
📌 Clinical Case
A 41-year-old woman (height: 1.68 m) sustained blunt chest trauma with pulmonary contusion after a 4-meter fall. She remains on mechanical ventilation and meets criteria for ARDS, for which a lung-protective strategy has been initiated.
While on PCV mode, the ventilator triggered the low tidal volume alarm.
In this exercise, you will:
✅ Adjust VT alarm limits according to predicted body weight and lung-protective ventilation recommendations.
✅ Discuss how proper alarm settings enhance safety and support early detection of ventilatory changes.
✅ Develop clinical reasoning and decision-making skills when responding to VT-related alarms.
